An extension is cheaper per square metre than a new build because it borrows services, a roof and a kitchen from a house that already exists. It is harder to detail, because everything it borrows has to be joined to.

Why extending beats building new, per square metre
The fixed costs of a dwelling — plan approval, professional fees, municipal connections, a kitchen, a bathroom, a roof structure, a site establishment — do not scale down with size. That is why a small new building is proportionally the most expensive thing to build.
An extension avoids most of them. The services exist. The kitchen exists. The connections exist. You are buying floor area and the junctions to attach it, which is a much shorter list.
That advantage is largest where the extension is simple: a rectangular addition on the flat side of the house, off an existing wall, with no new bathroom and no relocation of drainage.
It shrinks fast when the extension gets complicated. A new bathroom means a soil connection. A second storey means checking existing foundations. A wraparound on a sloping side means retaining and fill. Each of those pulls the per-square-metre cost back toward new-build territory.
So the first design question is not what you want, it is what the existing house makes easy. Extending in the direction the drainage already runs, off a wall that is not load-bearing in an awkward way, on the flat part of the erf, is dramatically cheaper than the same area somewhere else.
Approvals and what the erf allows
An extension needs approved plans, without exception, and the envelope is set before design starts.
Zoning and the title deed govern coverage, floor area ratio, building lines and height. You may have less room than you think, particularly on older subdivided erven with generous existing footprints.
Building lines are the constraint that most often kills the obvious extension direction. Building closer to a boundary than the scheme allows requires a departure application, which is a separate and slower process with no guaranteed outcome.

Joining old to new
This is where extensions are won or lost, and where a good one stops looking like an addition.
Foundations. The new foundation must be designed for the ground under it, which may not be the same as under the original house — particularly if the original was built decades ago to a different standard. Where new work abuts old, the two will settle differently no matter what, so the junction is either designed to accommodate that movement or it cracks. On active clay this matters a great deal.
Floor levels. Matching an existing floor level exactly requires knowing what the existing build-up is, and on an older house nobody does until it is opened up. A deliberate step is honest; an accidental 15 mm mismatch across a doorway is a trip hazard and looks like a mistake.
The wall junction. New masonry tied into old with proper wall ties or a movement joint, depending on the design intent. Plaster carried across a junction between two structures that will move differently will crack along that line, reliably.
The roof junction. The hardest detail on most extensions. A new roof meeting an existing one creates a valley, an abutment or both, and those are exactly where roofs leak. Matching pitch and covering matters aesthetically; getting the flashing and valley detailing right matters structurally.
Matching materials. Brick and tile colours change over decades and an exact match is usually impossible. The honest options are to accept a visible difference, to render the whole thing, or to design the junction so the change reads as deliberate.
Where the money goes, and how to keep it down
Drainage is the biggest single lever. An extension that needs a new soil connection costs substantially more than one that does not. If a bathroom is in the plan, put it where the drainage already is if the layout allows.
The roof junction is the second. A simple lean-to off a single wall is far cheaper than an extension that requires reworking the existing roof.
Existing structure checks. Adding a second storey means proving the existing foundations and walls can carry it, which means opening up and an engineer. Sometimes the answer is that they cannot, and that changes the project entirely.
Making good. Every extension leaves edges — plaster, floors, skirtings, paint that will not match so the whole room gets painted. Consistently underestimated.
Living in it. Working around an occupied house is slower and needs protection and daily clean-down.
Ways to keep it down: keep the shape simple and rectangular; extend in the direction the services already run; avoid a second storey unless you have to; use lighter construction where the ground is difficult, since a framed extension puts far less load on foundations; and make every selection before work starts.
