Most people who die in confined spaces are not the person who entered first. They are the ones who went in to help, without equipment, because there was no rescue plan.
Confined Space Entry: What the Regulations Require — practical guidance from the team running confined space entry on the Garden Route. Below is what legally counts as a confined space, what must be in place before entry, and why the standby person and the rescue arrangement matter more than anything else on the list.
What counts as a confined space
The definition is functional rather than about size, and that is what catches people out.
A confined space is an enclosed or partially enclosed space that is not designed for continuous human occupancy, has restricted means of entry and exit, and in which a hazardous atmosphere may accumulate or a person may be at risk of engulfment or entrapment.
Common examples on industrial sites: tanks and vessels, boilers, silos, sewers and manholes, pits and sumps, ducts, tunnels and culverts, and the inside of process equipment.
Less obvious ones: an excavation deep enough for heavier-than-air gases to collect; a partially enclosed structure; a space that only becomes hazardous because of the work being done in it, such as welding, coating or solvent use.
The atmosphere is the killer. Oxygen deficiency, oxygen enrichment, flammable atmospheres and toxic gases. Several of them give no warning at all — hydrogen sulphide deadens the sense of smell at exactly the concentrations that matter, and an oxygen-deficient atmosphere produces unconsciousness with no sensation of breathlessness.
And the atmosphere changes. A space that tested safe in the morning can be lethal by the afternoon because of work done in it, because sludge was disturbed, or because something drained in from a connected system.
What must be in place before entry
A risk assessment specific to that space and that task.
An entry permit — a written authorisation issued by a competent person, valid for a defined period and set of conditions, listing the atmospheric test results, the controls in place, the people authorised to enter and the rescue arrangements. A permit is a decision, not a form.
Isolation. Every energy source and every connected system physically isolated and locked out — electrical supply, mechanical drives, and crucially any pipework that could deliver liquid, gas or steam into the space. Blanking or double-block-and-bleed rather than a closed valve, because a closed valve can be opened.
Cleaning and purging where residues could give off vapour.
Atmospheric testing before entry — oxygen, flammability and toxics — with a calibrated instrument, testing at different depths because gases stratify. Then continuous monitoring during the work, because conditions change.
Ventilation, usually forced, maintained throughout.
Communication between the entrant and the outside that works in that space.
A standby person stationed at the entry point, and a rescue arrangement.
The standby person and the rescue arrangement
These two items are where confined space fatalities concentrate, and they deserve stating plainly.
The standby person stays outside. Always. Their role is to maintain communication, monitor conditions, control who enters, and raise the alarm. They do not enter to help. That single rule, held, is what prevents most multiple-fatality incidents — and it is broken by instinct, which is exactly why it has to be trained, briefed and enforced rather than assumed.
The rescue arrangement must be non-entry wherever possible. A harness and retrieval line attached to the entrant, running to a tripod and winch at the opening, so a casualty can be extracted without anyone going in. Set up before entry, not fetched afterwards.
Where non-entry rescue is not possible because of the geometry, an entry rescue team must be present, equipped with breathing apparatus, and trained for that specific space.
“We will call emergency services” is not a rescue arrangement. Response times do not match the physiology — in an oxygen-deficient atmosphere, unconsciousness comes in minutes.
Ask when the rescue arrangement was last practised on a comparable space, and ask for the record. It is the most informative question you can put to a confined space contractor.
Where rope access fits
Rope access and confined space work overlap constantly on industrial sites, and the combination is often the safest available option.
Vertical confined spaces — silos, vessels, shafts, chimneys — require controlled descent and, more importantly, controlled retrieval. A rope access team is already equipped and trained for exactly that, which makes non-entry and entry rescue substantially more achievable than with a crew whose only plan is a tripod at the top.
The competency requirements stack. A technician entering a confined space on rope needs rope access certification, confined space entry training, breathing apparatus training where applicable, and the trade competency for whatever work is being done — inspection, welding, coating, NDT. Four separate sets of evidence, and a contractor should be able to produce all four for each named person.
The rescue plan has to cover the combination, which is harder than either alone: recovering a casualty who is both suspended and inside a restricted space.
What to ask for before mobilisation: the entry permit procedure, the isolation certificate arrangement, the atmospheric monitoring equipment and its calibration records, the named standby person, the rescue method for that specific geometry, and the four sets of competency evidence per technician.

Recovering a casualty without anyone entering the space — typically a harness and retrieval line on the entrant, running to a tripod and winch at the opening, rigged before entry rather than fetched afterwards. It is the preferred arrangement wherever the geometry allows, because it removes the exposure that kills rescuers. Where it is not possible, an entry rescue team with breathing apparatus, trained for that specific space, must be on site.
Is atmospheric testing needed if the space was tested yesterday?
Yes. Atmospheres change — because of work done in the space, because sludge or residue was disturbed, because something drained in from a connected system, or simply over time. Testing is done immediately before entry, at different depths because gases stratify, with a calibrated instrument, and then continuously during the work. A test result from a previous shift tells you nothing about now.
What competencies should a confined space rope access team have?
Four separate sets of evidence per person: rope access certification at the appropriate level, confined space entry training, breathing apparatus training where the work requires it, and the trade competency for the actual work — welding, coating, NDT or inspection. Ask for all four, for the named individuals who will attend, with current dates. A contractor who can only produce two of the four is not equipped for the job.
